- Academic Results: Candidates should submit the academic transcript of their senior high school years.
- Statement of purpose.
- Learning Outcomes Assessment: Candidates should provide the learning outcomes (e.g., written reports or practical works) in the relevant areas according to their senior high school syllabus.
- Miscellaneous Performance: Candidates may provide the data of performance in other areas helpful for acceptance. Such data may include the performance in extracurricular activities, class or school club committee experience during senior high (vocational) school, service learning, competitions, licenses and certificates, distinguished performance, and other off-campus events.
